  4. Coordinate care with other healthcare providers, such as physicians and therapists, to ensure that patients receive comprehensive and effective care.

Alaskan Home Health Physical Therapy

Physical Therapy

Services may include:

  1.  Evaluation and assessment of patients' physical abilities and limitations, including range of motion, strength, balance, and mobility.

  2. Helping patients improve their walking abilities and teach them how to use assistive devices such as canes or walkers.

  3. Use various techniques such as manual therapy, heat or ice therapy, and electrical stimulation to help patients manage pain.

  4. Assessment of patients' homes for safety hazards and make recommendations for modifications to prevent falls and other injuries.

Alaskan Home Health Occupational Therapy

Occupational Therapy

Services may include:

  1. Evaluation and assessment of patients' ability to perform activities of daily living (ADLs) such as dressing, bathing, and grooming, as well as instrumental activities of daily living (IADLs) such as meal preparation, housekeeping, and medication management.

  2. Developing personalized treatment plans based on patients' individual needs, goals, and abilities, and use evidence-based interventions to help patients regain their independence.

  3. Providing cognitive and perceptual training to help patients improve their memory, attention, problem-solving, and visual-perceptual skills.

  4. Teaching patients how to conserve their energy and simplify tasks to make them more manageable.

Our Referral Process

Alaskan Home Health, Inc.

Eligibility

  • Patient must have the Traditional Medicare insurance with active Medicare Part A, and it should be the primary payer

  • Has a medical condition that requires part-time or intermittent skilled nursing care or therapy services

  • Homebound

  • Resides in Anchorage or Eagle River area

  • Must have a healthcare provider in the area that will follow home healthcare and sign agency orders

Referral

  • Healthcare provider ordering home health must be PECOS-enrolled

  • Healthcare provider writes, signs, and sends referral order and relevant medical records to our agency

  • Referral will be screened based on insurance, needs, and staffing availability

  • Notification of referring healthcare provider and patient regarding acceptance or denial of home health referral

Scheduling

We are required by Medicare to see the patient within 48 hours from the day we received the referral order or the patient has been discharged from the hospital.

We are transparent about our availability and we ensure that the provider and patient are informed for any delay or changes.

If we are unable to reach a patient despite multiple attempts, we inform the provider and cancel the referral.
