Our Referral Process

Eligibility

Patient must have the Traditional Medicare insurance with active Medicare Part A, and it should be the primary payer

Has a medical condition that requires part-time or intermittent skilled nursing care or therapy services

Homebound

Resides in Anchorage or Eagle River area

Must have a healthcare provider in the area that will follow home healthcare and sign agency orders

Referral
-
Healthcare provider ordering home health must be PECOS-enrolled
-
Healthcare provider writes, signs, and sends referral order and relevant medical records to our agency

Referral will be screened based on insurance, needs, and staffing availability
-
Notification of referring healthcare provider and patient regarding acceptance or denial of home health referral

Scheduling
We are required by Medicare to see the patient within 48 hours from the day we received the referral order or the patient has been discharged from the hospital.
We are transparent about our availability and we ensure that the provider and patient are informed for any delay or changes.
If we are unable to reach a patient despite multiple attempts, we inform the provider and cancel the referral.
